Key takeaways
- All 4 Paramount Elite plans sold to individuals in Wood, Lucas and Monroe County in 2026 are renewals with a service area reduction in the CMS plan crosswalk for 2027, and none keeps the Paramount Elite name for 2027.
- In Wood and Lucas County, each plan is moved on January 1, 2027 into a matching MedMutual Advantage Elite plan from the same parent company, Medical Mutual of Ohio. You do not have to re-enroll.
- Three of the four plans carry a higher yearly out-of-pocket maximum for 2027, such as $4,600 to $5,200 for Standard; the Preferred PPO stays at $5,300.
- In Monroe County, Michigan, the CMS Contract Year 2027 landscape lists no Medical Mutual Advantage plan, so plan to choose a new plan yourself.
- Choose by December 7, 2026 for coverage on January 1, 2027. A non-renewal Special Enrollment Period then runs through the last day of February 2027.
Are Paramount Elite Medicare plans leaving the Toledo area for 2027?
The Paramount Elite name is going away, and what happens to you depends on your county. The CMS plan crosswalk for 2027 (the file CMS publishes each fall showing what becomes of every plan) lists all four Paramount Elite plans sold to individuals in Wood, Lucas and Monroe County as “Renewal Plan with SAR,” CMS shorthand for a renewal with a service area reduction. In Wood and Lucas County, each plan moves into a plan with a new name from the same parent company, Medical Mutual of Ohio. In Monroe County, Michigan, the 2027 landscape lists no Medical Mutual Advantage plan at all.
Paramount Elite Medicare Plans is the Medicare Advantage brand of Paramount Care, Inc. (contract H3653) and Paramount Insurance Company (contract H5232), both listed under the parent organization Medical Mutual of Ohio in the CMS Contract Year 2026 landscape. A Medicare Advantage plan (Part C) is a private plan that replaces Original Medicare for hospital and doctor coverage and usually includes drug coverage (Part D). When a plan’s contract changes, its costs, drug list and network can change with it.
This page covers Wood County (FIPS 39173), Lucas County (FIPS 39095) and Monroe County, Michigan. Every number below was pulled from the CMS files on November 5, 2026, and each table names its source. For the short version, read what happens to my Paramount Elite plan in 2027. Next step: bring your Annual Notice of Change to Kris, who matches your plan to the tables below.
Which Paramount Elite plans are affected, and where does CMS move members?
Four individual plans, the same four in all three counties. CMS also lists employer-group versions of Paramount Elite, which are not sold to the public and are not covered here.
Each individual plan has one successor. The 2027 plans keep the same plan type: the three HMO-POS plans stay HMO-POS and the PPO stays a PPO.
| 2026 plan (contract-plan ID) | Type | 2027 plan (contract-plan ID) | CMS crosswalk status |
|---|---|---|---|
| Paramount Elite Enhanced (HMO-POS) (H3653-004) | HMO-POS | MedMutual Advantage Elite Enhanced (HMO-POS) (H6723-007) | Renewal plan with service area reduction |
| Paramount Elite Standard (HMO-POS) (H3653-015) | HMO-POS | MedMutual Advantage Elite Standard (HMO-POS) (H6723-008) | Renewal plan with service area reduction |
| Paramount Elite Prime (HMO-POS) (H3653-022) | HMO-POS | MedMutual Advantage Elite Prime (HMO-POS) (H6723-009) | Renewal plan with service area reduction |
| Paramount Elite Preferred (PPO) (H5232-001) | PPO | MedMutual Advantage Elite Preferred (PPO) (H4497-006) | Renewal plan with service area reduction |
Source: CMS PlanCrosswalk2027 (plan year 2026 to 2027), contracts H3653 and H5232, read November 5, 2026. The crosswalk is by plan, not by county; the county tables below show where each successor is actually offered.
An HMO-POS plan generally asks you to use the plan’s network for most care. A PPO lets you see out-of-network providers for a higher cost. Ask Kris how each plan treats your own doctors.
How do the 2027 costs compare with 2026?
Three of the four plans cost more in at least one column for 2027, and none gets cheaper on the numbers below. The Preferred PPO keeps the same premium, drug deductible and out-of-pocket limit; the other three each change at least one. The “out-of-pocket max” is the most you pay in a year for covered in-network medical care before the plan pays 100%. It does not count your monthly premium or most drug costs.
The Standard plan keeps a $0 premium, but its limit rises $600. Enhanced goes up $1 a month and $300 on the limit. Prime’s limit rises $250.
| 2027 plan | Monthly premium 2026 → 2027 | Part D deductible 2026 → 2027 | Out-of-pocket max 2026 → 2027 | CMS overall rating 2026 → 2027 |
|---|---|---|---|---|
| MedMutual Advantage Elite Enhanced (HMO-POS) | $74 → $75 | $55 → $55 | $3,700 → $4,000 | 3.5 stars → 3.5 stars |
| MedMutual Advantage Elite Standard (HMO-POS) | $0 → $0 | $225 → $380 | $4,600 → $5,200 | 3.5 stars → 3.5 stars |
| MedMutual Advantage Elite Prime (HMO-POS) | $35 → $35 | $250 → $250 | $4,200 → $4,450 | 3.5 stars → 3.5 stars |
| MedMutual Advantage Elite Preferred (PPO) | $0 → $0 | $300 → $300 | $5,300 → $5,300 | Not rated → 4.5 stars |
Source: CMS CY2026 landscape and CY2027 landscape (premium is the monthly Part C and D premium; out-of-pocket maximum is in-network), read November 5, 2026. The Preferred PPO had no rating in 2026; the 2027 landscape lists 4.5. Every year, Medicare evaluates plans based on a 5-star rating system.
- Enhanced 2026 $3,700
- Enhanced 2027 $4,000
- Standard 2026 $4,600
- Standard 2027 $5,200
- Prime 2026 $4,200
- Prime 2027 $4,450
- Preferred 2026 $5,300
- Preferred 2027 $5,300
Three of four plans carry a higher limit in 2027. A higher limit does not mean you will spend it; it is the ceiling if you have a serious illness or injury.

Does drug coverage change under the new Medical Mutual plans?
Yes, in two ways. First, the federal Part D rules reset. Under the CMS CY2027 Rate Announcement, the yearly out-of-pocket cap on covered drugs rises from $2,100 in 2026 to $2,400 in 2027, and the most any Part D plan can charge as a deductible rises from $615 to $700. All four plans sit below that $700 limit.
Second, the plans set their own deductibles and drug lists. Only the Standard plan’s drug deductible changes, from $225 to $380. The premium can also be split differently: in 2026 the landscape file shows Prime’s whole $35 as the medical part, and in 2027 as the drug part. Enhanced’s $75 for 2027 is $58 medical and $17 drug. The total is what you pay, but if you get Extra Help with drug costs, ask Kris what you would really pay.
- Enhanced 2026 $55
- Enhanced 2027 $55
- Standard 2026 $225
- Standard 2027 $380
- Prime 2026 $250
- Prime 2027 $250
- Preferred 2026 $300
- Preferred 2027 $300
Only the Standard plan’s deductible rises. Many plans charge a reduced copay on some drug tiers before the deductible, so the deductible alone does not set your drug bill.

What happens if you do nothing in Wood or Lucas County?
You are moved into the matching Medical Mutual plan on January 1, 2027, with no gap in coverage and no need to re-enroll. Your Paramount Elite plan ends on December 31, 2026, and the successor in the first table starts the next day. You will get a new member ID card, and the plan name on it will say MedMutual Advantage Elite.
Doing nothing is a real choice, and for many people it will be fine. The risk is not checking: a new contract can bring a different network, drug list and copays, even when the plan type is the same. Your Annual Notice of Change (the letter every plan member gets each fall) spells out the 2027 copays. Kris’s own answer in her Medicare FAQ about keeping your doctors is “You usually can. It’s important to be sure your doctor accepts Medicare. Some don’t.” With a new plan, the same check applies to its network: look up each doctor and hospital by name.
If you stay and the new plan does not work out, the Medicare Advantage Open Enrollment Period, January 1 to March 31, 2027, lets you make one switch to another Medicare Advantage plan or back to Original Medicare. That is a backup, not a plan.
What is different for Monroe County, Michigan members?
Monroe County members should not expect to be moved. The same four Paramount Elite plans were sold in Monroe County in 2026, at the same premiums, deductibles and out-of-pocket limits as in Ohio. The CMS Contract Year 2027 landscape has no row for Medical Mutual’s MedMutual Advantage contracts (H6723 and H4497) in Monroe County, Michigan. That fits the “service area reduction” label in the crosswalk: the plans continue in a smaller area, and Monroe County is outside it.
The 2027 landscape shows 44 Medicare Advantage plans with drug coverage in Monroe County, down from 45 in 2026. (That count leaves out other plan types, so it is not comparable to the Wood and Lucas totals.) Your plan’s notice is the final word, so read it first. If it says your plan is ending, choose a new plan under the same December 7 and end-of-February windows.
| 2026 plan sold in Monroe County | Monthly premium | Part D deductible | Out-of-pocket max | Medical Mutual plan listed in Monroe County for 2027 |

What other Medical Mutual plans are offered in Lucas County for 2027?
The Lucas County landscape also lists four other Medical Mutual plans with drug coverage that were not part of the Paramount crosswalk. We pulled Lucas County only for this table, so check Wood County availability with Kris.
Medical Mutual of Ohio is one of the carriers Kris represents. Across all companies, the landscape shows 76 Medicare Advantage plans with drug coverage in Lucas County and 72 in Wood County for 2027, 45 and 43 of them with a $0 monthly premium. So you are not limited to the plan CMS picks for you.
| 2027 plan (contract-plan ID) | Monthly premium | Part D deductible | Out-of-pocket max | CMS overall rating |
|---|---|---|---|---|
| MedMutual Advantage Classic (HMO-POS) (H6723-001) | $0 | $315 | $5,500 | 3.5 stars |
| MedMutual Advantage Select (PPO) (H4497-001) | $58 | $95 | $6,900 | 4.5 stars |
| MedMutual Advantage Preferred (PPO) (H4497-002) | $90 | $55 | $5,900 | 4.5 stars |

What are your deadlines?
You have two windows. The Annual Enrollment Period runs October 15 to December 7, 2026. Anything you choose in that window starts January 1, 2027, so there is no gap. That is the window to aim for.
Because your plan is changing contracts, you may also qualify for a Special Enrollment Period from December 8, 2026 through the last day of February 2027, according to CMS’s Special Enrollment Periods page. It is a safety net, not a plan: a choice made in January generally starts the first of the next month. A gap of 63 days or more without drug coverage can lead to a Part D late enrollment penalty, with some exceptions. The enrollment window tool shows your dates.
| If you choose a new plan… | Window | New coverage starts |
|---|---|---|
| Between October 15 and December 7, 2026 | Annual Enrollment Period | January 1, 2027 (no gap) |
| Between December 8 and December 31, 2026 | Special Enrollment Period (non-renewal) | Generally January 1, 2027 |
| Between January 1 and the last day of February 2027 | Special Enrollment Period (non-renewal) | Generally the first day of the month after you enroll |
| January 1 to March 31, 2027, if you are in a Medicare Advantage plan | Medicare Advantage Open Enrollment Period | The first of the month after you switch (one switch) |

Should you stay, switch plans or go back to Original Medicare?
You have three paths: stay in the Medical Mutual plan CMS assigns (Wood and Lucas County only), choose another Medicare Advantage plan, or use Original Medicare with a Medicare Supplement and a separate Part D plan. The right one depends on your doctors, your drugs and how much you want to budget for surprises.
Many people think Medicare Advantage works like a Medicare Supplement. It does not. A Medicare Supplement (Medigap) works with Original Medicare, lets you see any provider that accepts Medicare, and needs a separate Part D plan. You usually have to answer health questions to buy one. If you have both Medicare and Medicaid, read about Special Needs Plans first.
When your Medicare Advantage plan stops giving care in your area, you have a guaranteed issue right to buy certain Medigap policies without health questions, as early as 60 days before and no more than 63 days after your Medicare Advantage coverage ends, per the Medicare guide Choosing a Medigap Policy (pages 15 to 18). It applies if you switch to Original Medicare, not if you join another Medicare Advantage plan. That window is short and does not come back, so Kris checks it with you before you decide. Read the Medigap vs. Medicare Advantage comparison and the Learning Center guide for the trade-offs.
What should you check before December 7?
Start now, not in December. (Not sure what your letter means? Read got a letter saying your Medicare plan is ending.) Here is the order Kris works through with clients whose plan is changing:
- Find your Annual Notice of Change and your current Paramount Elite card. The plan name tells you which row in the first table is yours.
- Write down every doctor, specialist and hospital you use, including ProMedica or Mercy Health locations. Networks differ by plan, so each name gets checked.
- Make a drug list with doses, then check it with the Rx drug lookup. Drug lists and tiers are the biggest cost difference between plans.
- Compare the out-of-pocket maximum along with the premium. A $0 premium with a $5,200 limit is a different plan than a $75 premium with a $4,000 limit.
- Decide whether you want Medicare Advantage or Original Medicare with a Medicare Supplement, and ask about the Medigap guaranteed issue window if you are leaving Medicare Advantage.
- Enroll by December 7, 2026. If you live in Monroe County, Michigan, treat this as required, not optional.
She checks your doctors and prescriptions against each plan she represents, compares them side by side, and fills out the application with you, at the Perrysburg office or at your home. Request a comparison or call (419) 277-8097. See also the guide to checking doctors and drugs and the MediGold exit post.
