Medicare’s negotiated price is $197.00 for a 30-day supply of Jardiance and $178.50 for Farxiga, down from list prices of $573 and $556, in effect since January 1, 2026 under the Medicare Drug Price Negotiation Program. What you pay in 2027 is your plan’s copay or coinsurance, and your out-of-pocket spending on covered Part D drugs is capped at $2,400 for 2027.
This applies to Jardiance (empagliflozin) and Farxiga (dapagliflozin) through a Part D plan or a Medicare Advantage plan with drug coverage. Your cost depends on the tier your plan puts each drug on and whether you have met your deductible (up to $700 in 2027).
How it works: the negotiated price, the tier and the cap
The negotiated price is what your plan pays for the drug; it is not your bill. Your share comes from the tier your plan assigns. A tier is the price level a plan puts a drug on, and it usually means a set copay or a percentage of the price (coinsurance). Both drugs were among the first ten that Medicare negotiated, so the plan’s cost fell sharply in 2026, as the spending data below shows.
| Measure | Jardiance | Farxiga |
|---|---|---|
| CMS negotiated price, 30-day supply, 2026 | $197.00 | $178.50 |
| List price, 30-day supply | $573.00 | $556.00 |
| Discount from list price | 66% | 68% |
| People with Part D who used it, 2025 | 2,977,956 | 1,421,744 |
| Average Part D spending per prescription, 2025 | $1,091.19 | $997.75 |
| Average Part D spending per prescription, January to March 2026 (negotiated price in effect, quarterly release) | $386.83 | $328.97 |

- 2025 (full year) $1,091.19
- 2026 (Jan to Mar) $386.83
The drop reflects the negotiated price taking effect on January 1, 2026.

What it costs you in 2027
Until you meet your plan’s deductible (up to $700 in 2027; some plans have less or none), you may pay the plan’s price for the drug. After that, you pay the tier copay or coinsurance. Once what you pay out of pocket for covered drugs reaches $2,400 in 2027, you pay $0 for covered Part D drugs for the rest of the year.
If you get Extra Help, you pay a small fixed copay for brand drugs instead. If large costs early in the year are hard on your budget, the Medicare Prescription Payment Plan lets you spread them into monthly payments through your plan. Ask for it when you enroll.
We have not loaded 2027 formulary tiers for these two drugs yet, so this page does not say how many plans cover them or on which tier. That is the first thing Kris checks in each plan.
What to do next
Send Kris your full drug list with doses and your pharmacy. She checks where Jardiance or Farxiga sits in each 2027 plan she represents and your total yearly cost, not just the premium, and enrolls you by December 7, 2026. You can also use the Rx drug lookup form.
