Medicare's negotiated price for Eliquis was set at $231 for a 30-day supply for 2026, down from a $521 list price, in effect since January 1, 2026 under the Medicare Drug Price Negotiation Program and adjusted each year for inflation. What you pay in 2027 is your plan's copay or coinsurance, and what you pay out of pocket for covered Part D drugs is capped at $2,400 for 2027.
This applies to Eliquis (apixaban) through a Part D plan or a Medicare Advantage plan with drug coverage. Your cost depends on the tier your plan puts it on and whether you have met your deductible (up to $700 in 2027).
How it works: the price, the tier and the cap
The negotiated price is what your plan pays. Your share comes from the plan's tier for Eliquis. A tier is the price level a plan puts a drug on; tier 3, "preferred brand", usually means a set copay or a percentage of the price (coinsurance). About 9 in 10 plans put Eliquis on tier 3. Our count below covers every renewing Medicare drug plan nationwide, from the CMS formulary files:
| Measure | Figure |
|---|---|
| People with Part D who used Eliquis, 2025 | 4,806,752 |
| Average Part D spending per prescription, 2025 | $912.31 |
| Average Part D spending per prescription, January to March 2026 (negotiated price in effect, quarterly release) | $399.01 |
| 2027 plans nationwide with Eliquis on tier 3 (preferred brand) | 4,287 of 4,748 (90%) |
| 2027 plans with Eliquis on tier 2 or tier 1 | 461 of 4,748 (10%) |
| Renewing plans that dropped Eliquis for 2027 | 0 |

What it costs you in 2027
It works in three steps. Until you meet your plan's deductible (up to $700 in 2027; some plans have less or none), you may pay the plan's price for Eliquis. After that, you pay the tier copay or coinsurance. Once what you pay out of pocket for covered drugs reaches $2,400 in 2027, you pay $0 for covered Part D drugs for the rest of the year.
The table shows average plan spending per prescription before manufacturer rebates, including 90-day fills, so it is not what you pay. If you get Extra Help, you pay a small fixed copay for brand drugs like Eliquis instead.
If big costs early in the year are hard on your budget, the Medicare Prescription Payment Plan lets you spread them into monthly payments through your plan. Ask for it when you enroll.

Send Kris your full drug list with doses and your pharmacy. She checks the Eliquis tier and your total yearly cost in each 2027 plan she represents, not just the premium, and enrolls you by December 7, 2026. You can also use the Rx drug lookup form.
