Choose a new plan by December 7, 2026 and it starts January 1, 2027 with no gap. All 4 UnitedHealthcare Dual Complete Special Needs Plans sold in Lucas and Wood County in 2026 are terminated for 2027 with no successor plan, according to the CMS 2027 plan crosswalk.
This applies to people with both Medicare and Medicaid (dual-eligible) in a UHC Dual Complete D-SNP in Lucas or Wood County, Ohio. You keep Medicare and Medicaid. What ends is the plan that coordinated them, along with its drug coverage and extra benefits.
How it works: which UHC plans are ending
A D-SNP is a Medicare Advantage plan only for people who also have Medicaid. These four are ending in both counties:
| 2026 plan (ID) | Type | 2027 status (CMS) | Successor plan |
|---|---|---|---|
| UHC Dual Complete OH-S3 (H1285-002) | HMO-POS D-SNP | Terminated / non-renewed | None |
| UHC Dual Complete OH-D1 (H2001-058) | PPO D-SNP | Terminated / non-renewed | None |
| UHC Dual Complete OH-D002 (H5322-028) | HMO-POS D-SNP | Terminated / non-renewed | None |
| UHC Dual Complete OH-V002 (H5322-034) | HMO-POS D-SNP | Terminated / non-renewed | None |

What it changes: your options for 2027
You are not alone: the CMS 2027 plan crosswalk lists 13 D-SNPs from five companies in each county as terminated for 2027. The CMS CY2027 landscape still lists 11 D-SNPs in each county, from Anthem (4), Wellcare (4), Molina (2) and CareSource (1), 4 of them new or newly combined this year. Which one you can join depends on your Medicaid level (full or partial benefits), so match the H-number on your card to the table above first.
If you do nothing, you go to Original Medicare on January 1. If you have Medicaid or Extra Help, you are generally assigned a plan so you are not left without drug coverage (by Medicare, or in some cases through Ohio Medicaid), but an assigned plan may not cover your drugs or use your pharmacy, so it still pays to choose. People with Medicaid or Extra Help can also use a monthly Special Enrollment Period to switch to Original Medicare with a stand-alone drug plan, or into a plan that combines Medicare and Medicaid. Your Medicaid coverage itself does not end because the plan ends.
What to do next
Gather your Medicare card, your Medicaid card, your doctors and your prescriptions. Kris checks which 2027 D-SNPs she represents in your county, whether your doctors are in network, and how your drugs are covered, then helps you enroll. Read how Special Needs Plans work.
